What Do I Need to Bring to My First Appointment?

Your first visit is important, and bringing the correct information helps us provide you with the best care quickly. Please make sure to bring your complete medical history, noting any details about current or past medical conditions. Have your insurance information ready for verification. Provide a list of any medications you take daily, weekly, as needed, from prescription to over-the-counter, and note any allergies you have.  Be sure to bring a valid form of identification, such as a driver’s license. Having these ensures a smooth and efficient appointment process.
